Job Description:

Jersey Shore Post Acute is seeking a dedicated and compassionate Outpatient Physical Therapist to join our team. This role involves delivering personalized rehabilitation services to our patients, aiding in their recovery and enhancing their overall health and mobility. The successful candidate will receive a $2,500 sign-on bonus and join a facility committed to community health and innovative care practices. Evaluate patient conditions and establish treatment goals. Develop tailored treatment plans that adhere to individual recovery needs. Implement therapeutic interventions to improve patient mobility and functionality. Record and document patient progress and modify treatment plans as required. Educate patients and family members on aftercare practices. Collaborate with other healthcare professionals to optimize patient care. Strong understanding of physical therapy modalities and techniques. Ability to design effective, patient-specific therapy programs. Excellent interpersonal and communication skills. Proficient in using physical therapy equipment and tools. Medical Insurance Dental Insurance Vision Insurance Valid Physical Therapist license. CPR/BLS certification is preferred.
