Job Description Job Description OVERVIEW: The Resident Services Case Manager assesses resident's needs, develops individualized care plans, coordinate services (ex. health care, mental health, housing related issues, employment, education) manages documentation, maintains an active caseload, and advocates for residents living in West Brighton Houses, focusing on providing resources and self-sufficiency through client centered support. HOU's mission is to ensure that residents are treated fairly and experience minimum physical and emotional stress during the relocation process while ensuring that our clients' projects are completed in a timely and cost-effective manner. Core Responsibilities Assessment & Planning: Conduct intake, assess needs (physical, emotional, social) provide referrals and create individualized care plans adjusting as needed. Direct Support: Offer resident care counseling, crisis intervention, life skill trainings/workshops, and emotional support, assisting with daily living tasks if necessary Advocacy: Act as a resident advocate, ensuring their needs are met, rights are protected, and they receive quality, free or cost-effective care. Documentation, Monitoring & Evaluation: Maintain accurate, confidential case files, notes, and monthly reports, ensuring compliance. Communication: Maintain regular contact with resident caseloads, families, and the PACT Team, documenting all progress and obstacles. Crisis Intervention/Case Management: Provide ongoing support, referrals, resources, advocacy and crisis intervention. Key Skills & Qualifications Active listening, empathy, building rapport, collaboration, and effective communication with residents, families, and service providers. Critical thinking, resolving conflicts, crisis intervention, NYS Meet (through office and home visits) and establish working relationships with caseload heads of household's on-site residents 18 years of age and older, to partner and develop Individualized Services Plans. Ability to connect residents to appropriate job readiness activities (skill/ability assessment, resume building, basic computer skills, employment search resources, interview skills, dress for success). Ability to refer residents to educational opportunities, including college, specific skills training, etc., by maintaining contact with ESL and GED programs, college and university representatives, technical training schools, etc. Ability to connect families with needed and available community resources. Follow up with clients and agencies as appropriate to document use/ success of referral. Participate in weekly case management meetings and provide regular caseload progress reports to the Director of Resident Services and other team members. Attend case management meetings with agencies serving families on caseload, as Participate in all required meetings and staff training opportunities Input and maintain accurate and complete data for all residents in efforts and outcomes via HOU case management software Complete monthly reports timely Conduct office/home visits with residents on caseload Bilingual Spanish speaking a plus Other duties as assigned QUALIFICATIONS: BSW (required) or at least 10 years of related work experience in social work, human services or residential support Extensive experience working collaboratively with low-income families and community-based organizations. Problem-solving and strong administrative and organizational skills Proficient in Microsoft outlook, Excel, Google slides and case management software Excellent written and verbal communication skills; ability to create reports Ability to present virtually via Zoom, Microsoft Teams and Google Classroom A strong sense of respect for confidentiality involving both residents and fellow employees Valid NYS driver's license, reliable transportation, ability to work independently The ideal Resident Case Manager will possess the following additional skills: Organized and detail-oriented with the ability to prioritize multiple requests Flexibility and adaptability as challenging situations arise Ability to maintain a calm atmosphere under pressure and stressful situations Advocating for residents' rights Ability to maintain a calm atmosphere under pressure and stressful situations Patience, integrity, reliability, cultural competence, and maintaining confidentiality Sensitivity to different cultures and beliefs, with the capacity to work with diversity that extends beyond race.
