Job Description:
MedStar Health, a leading non-profit healthcare organization in
the DMV area, is seeking a dedicated and patient-oriented RN Case
Manager OH to join our team. In this vital role, you will be
responsible for coordinating comprehensive, patient-centered care
across the continuum of health services. You will advocate for
patients' needs, communicate with healthcare providers, and ensure
the effective delivery of health services. The ideal candidate will
be committed to enhancing patient outcomes and promoting the
cost-effectiveness of care, aligning with our mission to
continually improve health and well-being of the communities we
serve. Assess patient needs and identify appropriate medical,
social, and emotional support. Create and implement personalized
care plans in collaboration with healthcare professionals. Monitor
and evaluate patient progress, adjusting care plans as necessary.
Facilitate communication between patients, family members, and
medical staff. Ensure compliance with healthcare regulations and
maintain accurate patient records. Educate patients and families on
health conditions and care plans. Strong organizational and
multitasking skills. Excellent communication and interpersonal
abilities. Proficient in medical software and electronic health
records. Ability to work independently and as part of a team.
Medical Insurance Dental Insurance Vision Insurance Life Insurance
401k Current Registered Nurse (RN) license in the state of Ohio.
Certified Case Manager (CCM) credential preferred.
