Job Description:
Trinity Health At Home is seeking a dedicated and compassionate
Registered Nurse to join our team, providing high-quality medical
care to our patients in the comfort of their own homes. In this
role, you will work collaboratively with multidisciplinary team
members to develop and deliver personalized care plans, focusing on
promoting patient recovery and enabling patients with chronic
conditions to manage their health in their home environments. This
position requires a proactive approach to patient care and the
ability to adapt to the varied needs of our clients. Conduct
initial patient assessments and develop individualized plans of
care in collaboration with a multidisciplinary healthcare team.
Administer medications, perform routine treatments, and monitor
patient health status and progress. Educate patients and families
on health conditions and self-care skills. Ensure timely
documentation of patient interactions and interventions according
to applicable regulatory standards. Participate in on-call
rotations to provide 24-hour availability for high-priority cases.
Strong clinical skills with the ability to perform comprehensive
nursing assessments. Excellent communication and interpersonal
skills for interacting with patients, families, and healthcare team
members. Ability to stay organized and manage multiple patient care
plans effectively. Proficiency in utilizing healthcare technology
and electronic health records. Medical Insurance Dental Insurance
Vision Insurance Active Registered Nurse (RN) license in the state
of employment. CPR certification required. Certification in
advanced home health nursing is desirable.
