Join our dedicated team as a Registered Nurse (RN) Care Coordinator, providing essential medical oversight and compassionate care coordination to a meaningful population.Registered Nurse (RN) Care Coordinator - Behavioral HealthDescription: As the RN Care Coordinator you will provide direct supervision and coordination for all the medical services and health care needs of individuals receiving services.Schedule:

• Monday-Friday, 8:00 AM - 4:30 PM (Flexibility Required)
• On call every 3 months +Salary and Compensation:
  • $68,000 - $90,000 based on experience Benefits & Perks:

      • Registered Nurse (RN) or Licensed Practical Nurse (LPN) eligible.
      • Must hold a valid U.S. Driver's License in State of PA.
      • Minimum of 1 year experience in Charge Nurse, Supervisor RN in Behavioral Health, Psych or Mental Health setting.
      • Minimum of two (2) years' experience working with patients with Intellectual and Developmental Disabilities.
      • Annual Flu Vaccine required. Key Responsibilities:
        • Provide direct supervision and coordination of all medical services.
        • Travel to see patients living within community homes located in Delaware County, PA
        • Oversee health care needs of individuals with IDD/autism.
        • Collaborate with interdisciplinary teams to ensure high-quality care.
        • Maintain medical records, assist with care plans, and provide health education. Candidate Must Be:
          • Comfortable with commuting to various community homes.
          • Comfortable with CPI and supporting individuals with challenging behaviors.
