Job Description Job Description Degree in Physical Therapy,
active Physical Therapist license and 1 year of Physical Therapy
experience required. Applicants who do not meet these
qualifications will not be considered. A full-time Physical
Therapist role is available in a school-based setting serving
preschool and school-aged students for the 20252026 school year in
Middletown, New York. This contract position involves providing
specialized physical therapy services within educational
environments with a focus on supporting each students mobility,
strength, and functional independence. Location: Middletown, NY
Schedule: 32.5 hours per week | 20252026 school year Setting:
School-based Responsibilities: Deliver physical therapy
interventions aligned with Individualized Education Program (IEP)
goals and New York State Education Department (NYSED)/Department of
Education guidelines Assess and evaluate students physical needs
and create tailored treatment plans to improve motor skills and
functional abilities Collaborate with educators, support staff, and
families to facilitate student progress and generalize motor skills
across environments Participate in Committee on Special Education
(CSE) meetings, offering professional recommendations based on
student assessment Maintain detailed and accurate documentation and
progress reports in compliance with DOE and district standards
Qualifications: Masters or Doctoral degree in Physical Therapy from
an accredited institution Current New York Physical Therapy license
or eligibility to obtain licensure Experience in school-based or
pediatric physical therapy preferred Knowledge of CSE processes and
DOE requirements is desirable Strong skills in communication,
